Summit Case Management Group

About Us

Dedicated to rebuilding lives with care, clarity, and compassion

At Summit Case Management, we believe that recovery after a life-changing injury is more than just medical treatment — it’s about restoring independence, dignity, and purpose. We are passionate about providing high-quality, person-centered case management services that support individuals and their families through every step of the recovery journey.

Our Mission

To empower individuals with complex injuries — such as brain injuries, spinal cord injuries, and trauma — to live full and independent lives through expert coordination of care, rehabilitation, and support services.

Our Approach

We take a holistic, client-led approach to case management. This means:

Every plan we create is tailored to the unique circumstances of the individual, with the ultimate aim of improving quality of life and maximizing independence.

We are based in the South of England and work with clients across a wide geographical area. While our core network is established locally, our reach extends nationally where the needs of the case require it.

We believe that effective case management is not defined by postcode, but by the quality of the match between client and case manager. The right expertise, approach, and therapeutic fit are critical to achieving meaningful outcomes
